[PATREON POST + ART PACK] Hey everyone... I see March is rounding the corner to being over and I realized I never actually make an official patreon post yet. A lot has been happening on our end / IRL. We recently found our we might need to move this summer... But thanks to a lot of back and forth between us and the new landlord we got a lease extension for 1 year to prepare our savings and safely move out to another place in town or around this town we are currently living in. After a lot of discussion between my husband and I, we're really struggling to find rental single family homes that fit our disability needs AND budget. Rent bubble in our area is about 70% more than our current rent which... is mind blowing. But we are determined! With further discussion we think we agree, we'd like to buy a house to make sure our needs are met and that we can take care of our senior dogs without having our lives uprooted without notice. So our belts are going to be really tight over the next year while we try to scrimp and save every penny to go towards our goal to purchase a house next year. That is scary but also so exciting! Wish us luck, we're going to need every lick of it to pull this off. ~~~ Other worse news... The reason why I've been struggling these past few weeks. My family has been reaching out to me and it's official. My mother has a terminal cancer diagnosis. I do not live near her, so I would like to budget a trip out to see her at some point this summer. We don't have any real time lines yet until we see how to reacts to chemo treatment. She's just started that this month. So that's been a weird tangle of emotions and talking with my siblings about what we expect out of all this. How we each can help in what ways we can. One of my older sisters is taking control of talking with the doctors and updating us since my mother is extremely avoidant about talking about her illnesses. At this time I won't be taking any time off from working and doing adopts/commissions. We need the money more than ever, even just to go fly or drive up to see her. But I will update on patreon/discord when I'm taking a week or so off to visit her. ~~~ More middling news? Our two senior dogs, Ollie and Junior, have been having little health scares this year. Feels like 2024 just started and so much has happened! Ollie has been diagnosed with a heart murmur and is on medication now to help treat it. Diet and life style changes as well to help him stay fit. Junior may have had a small seizure or stroke, for 2 days he was not able to stand or walk which really scared us. Both have been to the vet multiple times this year and are being watched VERY closely. I just hope nothing happens to them before we find them a new house to live in. (knocks on wood) ~~~ I myself am having some medical issues but... Just with everything going on I just haven't had time to assign myself with a new clinic and new gp. My new health insurance won't let me see the old one anymore and it's EXTREMELY disruptive to my whole life. So there's a chance my body might get a flat tire in the future, but I'll try to manage it accordingly. To end it all on a good note, I did finally finish my very last tooth filling / replacement and after a year and a half of constant dentist visits, my mouth is all fixed! Hooray!!!! Now we just have to tackle my jaw and TMJ problems haha. ~~~ I think... that's the big items of what's going on. Why I've been really absent online and for updates. Life just kinda had a weird downpour on us, but we're sorting it out. We signed a new lease. Prov is working very hard at his new job. The future is really scary for me right now and I'm really struggling. Please be patient with me while we go through these big life changes at this time. I'll be opening up new commission slots soon to start a monthly income to help with house savings. I have a pt who has apparent status migrainosus. But it's been resistant to sumatriptan. She said she had some relief with IV meds in the ED (I believe she got depakote or valproate). I wonder if it might actually be 2/2 temporomandibular joint arthritis, which she was diagnosed with a month ago. That's what my preceptor mentioned. So I looked up TMD on UpToDate:
Pathogenesis – Several factors can contribute to the development of temporomandibular disorder (TMD) symptoms, including temporomandibular joint (TMJ) trauma, poor head and cervical posture, differences in pain threshold and processing, and psychological factors such as depression and anxiety.
●Clinical manifestations – Symptoms of TMD most commonly include facial pain: a dull, unilateral facial ache that is constant but waxes and wanes in intensity and is typically aggravated by jaw motion. Other common symptoms include earache, headache (typically frontal or temporal and often radiating to the jaw), and jaw and TMJ dysfunction (eg, decreased mandibular range of motion, clicking with jaw movement, intermittent jaw locking).
●Evaluation and diagnosis – The diagnosis of TMD is based primarily upon the history, including the patient's constellation of symptoms, and compatible physical examination findings. For patients with more severe symptoms and those with persistent symptoms, we obtain imaging to rule out local pathology in the teeth and jaw bones; a panoramic radiograph or cone beam computed tomography (CBCT) are both reasonable choices. We advise against the routine use of a standard radiograph of the TMJ, as this study does not provide valuable information regarding dentition and mandibular alignment and symmetry.
●Initial management with patient education and self-care measures – For all patients with TMD, we suggest initial management with education and self-care measures (Grade 2C). This includes education regarding the natural history of TMD and counseling on optimal head posture, jaw exercises (picture 2 and picture 3*), and proper sleep hygiene, as well as avoidance of triggers (eg, oral behaviors such as nail biting, pen chewing) if these factors contribute to symptoms. There are limited high-quality data supporting the use of self-care and education in the treatment of TMD; however, given the lack of harm and the potential benefit, these interventions are appropriate for all patients.
*Isometric jaw exercises are particularly useful for patients with temporomandibular joint dysfunction syndrome. These exercises are performed by applying resistance with an open or loosely fisted hand. In the isometric jaw opening exercise, the patient begins with her mouth open about an inch. The resistance and muscle contraction are held for 5 to 10 seconds before relaxing. This is repeated five times per session. Exercises can be performed with moderate resistance applied several sessions per day, or with maximum resistance one session per day.
*The isometric jaw forward thrust exercise is performed by pushing the jaw forward against the hand, holding and then relaxing. This is repeated five times per session.
•Physical therapy for musculoskeletal causes – For patients with a prominent musculoskeletal component or cervical or shoulder symptoms, we refer for physical therapy evaluation and treatment.
•Oclusal (bite) splints as adjunctive therapy – For some patients with TMD, particularly those with prominent musculoskeletal symptoms or evidence of bruxism, occlusal splints fitted by a dental clinician can be used as an adjunctive therapy along with other TMD treatments such as physical therapy.
•Biobehavioral management for comorbid psychological conditions – For patients with comorbid depression, anxiety, and stress disorders, biobehavioral management is used as adjunctive therapy in TMD treatment.
●Adjunctive pharmacotherapy for persistent symptoms – For patients with persistent symptoms despite education and self-care, we use adjunctive pharmacotherapy concurrently with other treatments.
•For such patients, we suggest using a nonsteroidal antiinflammatory drug (NSAID) as first-line pharmacologic therapy rather than other medications (Grade 2C). We generally treat patients with a 10- to 14-day course of a long-acting NSAID (eg, naproxen 250 to 500 mg orally twice daily).
•For patients with tenderness of the muscles of mastication, we suggest treatment with a skeletal muscle relaxant in addition to the NSAID (Grade 2C). For most patients, we prescribe the muscle relaxant for 10 to 14 days only. However, some patients with persistent muscular pain may benefit from an additional week of treatment.
•For patients with persistent TMD symptoms that warrant continued adjunctive pharmacotherapy after two weeks of NSAID treatment (or three weeks for those patients taking additional skeletal muscle relaxants), we suggest treatment with a tricyclic antidepressant (TCA) rather than other therapies (Grade 2C).
●Interventional management for severe or refractory symptoms – For patients with TMD symptoms refractory to noninvasive management, referral to an oral and maxillofacial surgeon with experience in managing TMD is warranted. Treatment options for these patients include trigger point muscle injections, botulinum toxin injections, and intraarticular injections.
●Surgical referral for refractory symptoms despite nonsurgical interventions – For some patients, such as those who have structural anatomic pathology on imaging and persistent jaw locking severe enough to interfere with activities of daily living despite three to six months of nonsurgical management, we suggest surgical intervention rather than no surgery (Grade 2C). Although the specific surgery offered will depend upon the intraarticular disorder, such patients are unlikely to achieve adequate symptom control without surgical intervention.

honestly i’m kind of pissed that orthodontics is something seen as necessary for kids, when it is very much 1) not and 2) comes with a lot of physical pain and care that a child cannot possibly consent to
I think ppl already know the usual arguments about how access to orthodontic care is very much class based. it sucks that teeth are treated as a status symbol. if u can’t afford it, you’re SOL. if you can afford it, you’re making a big commitment for your child’s healthcare and probably not explaining the whole process.
if I had known in middle school that I have to wear a retainer for the REST OF MY LIFE I probably would not have consented to getting braces. I get terrible tension headaches if I don’t wear it consistently, and jaw pain/clicking because whatever shifts they did to my under/overbite didn’t stay. this has been happening to me since I got the damn things off almost a decade ago, and i’m tired of dealing with chronic pain for what ultimately was a cosmetic procedure I didn’t choose for myself.
as far as I can tell, the only solution to this is going to an orthodontist and putting on braces AGAIN. i’ll be damned if I do that. I don’t have the money, or the insurance. also, they’re painful, they make me lisp, they’re ugly, and it would be embarrassing.
i’m trying to research into whether botox (used for migraine and TMJ pain) might also be a treatment available for the tension pain I get. it’s been irritating to research so far though because orthodontists’ websites populate the google search results, and they just want you to come in and get more damn braces. sigh. if anyone has experience dealing w this, help would be much appreciated

hey have you ever looked into tmj treatments? i honestly don’t really understand it so im not going to try to explain too much but basically i used to have what i call headaches but may have been migraines and nobody could help me and finally got referred to a tmj specialist who was like “oh your jaw is super misaligned, you’re grinding your teeth and it’s causing your headaches” and one of the things he mentioned is that like all the meds and the Botox that I was considering would’ve been essentially a bandaid for me personally—masking the symptoms but not fixing the problem. obviously don’t know if this applies to your migraines but especially since Botox has stopped being as effective for you (which sucks, im so sorry to hear that!!) i just wanted to mention it. sorry if any of my explainings didn’t make sense/was inaccurate, not trying to spread any falsehoods, this was just based on my experience cause i went from thinking my headaches were incurable to having these incredible retainers that are making my headaches go away almost entirely
i don't have any of the other tmj symptoms so i doubt it tbh!
